2. Would the $120 standard annual fee create a meaningful financial barrier to maintaining this required compliance support? No Yes

3. Confirm the program requirements The organization is, or expects to become, a California entity that needs an agent for service of process. I certify that these answers are accurate and understand that reduced pricing is provisional until reviewed.

Limited fee waivers may be available when funding permits. Fee-waiver requests are reviewed separately and are not automatically approved through this questionnaire.
